Each camper must have a comprehensive insurance policy in effect at all times when enrolled at Melody Pines. A copy of the insurance card for Camper (front and back) must be submitted with your application.
In addition, the State of New Hampshire REQUIRES that we have a statement on file, prior to your child's start at camp, signed by your physician and indicating the following:
- That your child has had a complete physical within the last 2 years (must be dated after Sept. 1st, 2017).
- That your child's immunizations are up to date.
- The date of your child's most recent tetanus shot.
If your doctor's office does not provide you with such a statement or record at your child's physical, please download the Universal Health Form for Camp and bring it to your doctor's office to be completed.
In addition, medication orders, signed by the parent/guardian and physician, must be provided in order for all prescription and non-prescription medications to be administered.
Failure to provide all required medical forms, at the time of registration, will result in the cancellation of your child's registration.
