- Parent/Guardian gives permission for Camper to be photographed by Melody Pines and for those photographs to be reproduced for any purpose that Melody Pines deems proper.

- This application has Parent/Guardian’s approval and consent. Parent/Guardian authorizes Melody Pines to act for Parent/Guardian according to Melody Pines best judgment in any emergency, including but not limited to having Camper treated by a certified physician.

- Parent/Guardian realizes fully that even after reasonable precautions are taken some activities such as but not limited to swimming, boating, water skiing, bb-guns and archery, may involve inherent risks for which Melody Pines cannot be held responsible and Parent/Guardian, on behalf of themselves and Camper, hereby releases Melody Pines from claims arising therefrom.

- Parent/Guardian has accurately completed this Application and has read, understands and agrees to the terms outlined herein. Parent/Guardian agrees to review all program materials sent and to complete all required forms. Parent/Guardian gives Camper permission to participate in all activities and programs at Melody Pines, unless Parent/Guardian has provided detailed written limitations to Melody Pines that have been accepted by Melody Pines. Parent/Guardian agrees that all aspects of this relationship with Melody Pines are governed by New Hampshire Law.

- Parent/Guardian and Camper agree to abide by all of the rules and regulations established by Melody Pines, from time to time, including without limitations, those relating to enrollment and withdrawal of Campers.

- Melody Pines reserves the right to dismiss, in its sole discretion, any Camper whose condition, conduct, influence or behavior is deemed unsatisfactory or detrimental to the best interests of Melody Pines or his/her fellow campers or who violates Melody Pines rules and regulations, in which case no refunds will be made.

- If tuition, fees or other monies due and owing to Melody Pines are not paid in full for Camper when due, Parent/Guardian will be liable for all costs of collection, including attorney’s fees and costs.

- Each Camper must have a comprehensive insurance policy in effect at all times while enrolled at Melody Pines. A copy of the insurance card for Camper (front and back) must be submitted with this Application. In addition, each Camper must provide a record of a current physical (within 2 years), as well as his/her immunization records. Compliance with this Section to the satisfaction of Melody Pines is an absolute prerequisite to each Camper’s acceptance at Melody Pines.

- In the event any claim is brought against Melody Pines, in which Melody Pines prevails, Melody Pines shall be entitled to an award equal to all attorneys’ fees, court costs and out-of-pocket expenses of Melody Pines and its staff.

- Parent/Guardian understands and acknowledges that this form is a legally binding New Hampshire Contract/Agreement.
