Each camper must have a comprehensive health insurance policy in effect at all times when enrolled at Melody Pines. A copy of the camper's health insurance card (front and back) must be submitted at the time of registration.
In addition, the State of New Hampshire REQUIRES that you submit the following to Melody Pines Day Camp in order for your child to attend:
- A copy of your child's most recent physical, complete physical within the last 2 years (must be dated after Sept. 1st, 2021).
- A copy of your child's immunization record indicating that your child's immunizations are up to date.
Copies of your child's health insurance card, physical, and immunization record are due at the time of registration. Failure to provide these required documents will result in the cancellation of your child's registration.
In addition, in order for all prescription and non-prescription medications to be administered, the parent/guardian must complete the Melody Pines Day Camp Permission to Administer Form along with current medication orders from the physician. All medications must be provided in their original prescription containers.
